Steps you took that leads to the bug

PM raised invoice
I approved request very quickly
PM thinks they clicked multiple times
This resulted in a duplicate request raised that has not been approved but can't be deleted

Expected outcome

only one request and I should be able to cancel the second

Actual outcome

a second request appears for the same one that I cannot remove
